Intake Form
Fill out the online intake form to get connected to a doctor. Our service is HIPAA-compliant, so all personal and medical information is completely confidential between you and your doctor. You will NOT be charged if the doctor believes you do not qualify for a medical card.
Talk to a Doctor
The next step is to visit with a doctor using any video-capable device from the comfort of your home. Our doctors will work with you to create a treatment plan and provide you with all the documentation necessary to get your medical card in the State of Ohio.
Register With the State
Once you have been certified by one of our doctors, you will receive an email. The email will contain the link you need in order to register online. You will have to pay the state registration fee of $50. Patients who qualify for indigent or veteran status can have their registration fee reduced by 50%. Once approved by the state, you should receive your card within 2 weeks.

You may be eligible for a Ohio medical card if you have been diagnosed with one or more of the following severe debilitating or life threatening conditions:

- Amyotrophic lateral sclerosis

- Chronic traumatic encephalopathy

- Epilepsy or another seizure disorder
- Fibromyalgia
- Glaucoma
- Hepatitis C
- Inflammatory bowel disease
- Multiple sclerosis
- Parkinson’s disease
- HIV
- PTSD
- Sickle cell anemia
- Spinal cord disease or injury
- Tourette syndrome
- Traumatic brain injury
- Ulcerative colitis
